Nucleus® Bimodal Update

More patients can now be bimodally fit than before; including our youngest and oldest patients.  The ReSound Up Smart pediatric hearing aid line, which features True Wireless™ connectivity; is one suite of hearing aids now included in Cochlear’s bimodal solution. Combining a ReSound Up Smart hearing aid with a Kanso® Sound Processor*, for example, can be used for the pediatric patient best-fit in a bimodal configuration.

For older children and adults who might be interested in Made for Apple® capabilities, using a Nucleus® 7 cochlear implant sound processor with any of the ReSound LiNX 3D or ENZO 3D technology level 7 hearing aids can allow them a premium level of connectivity.  This device pairing is compatible with both the Nucleus® Smart App and the ReSound Smart 3D App, giving parents and recipients access to powerful tools such as Find My Sound Processor and Hearing Tracker.  As always, to further enhance a patient’s hearing in everyday noisy situations, Cochlear’s True Wireless accessories: the Mini Microphone 2+, Phone Clip or TV Streamer are available.

New TECH Talk: Early implantation and its impact on language development

In March 2020 the FDA approved the first expansion of pediatric indications in 20 years. Dr. Jace Wolfe, Director of Audiology and Research at the Hearts for Hearing Foundation in Oklahoma City, analyzes the benefits of early implantation and its impact on brain development.

Wolfe highlights Jaime Leigh’s study1 that suggests the need to mitigate delay in implantation. For example, if a young child is implanted, that child can parallel one year of language progress in one calendar year without falling behind their peers.  

On the other hand, if a child with severe to profound hearing loss waits to be implanted, he or she can only make three months of progress for every calendar year without a cochlear implant.

Wolfe continues by addressing the variability in cochlear implant recipient outcomes. According to Leigh, age accounts for 67% in variability of outcomes in children with hearing loss.1 So, implanting as early as possible can be key, especially as pediatric patients reach hearing and language milestones.

Long-Term Outcomes of Cochlear Implantation in Patients With High-Frequency Hearing Loss

Objective: To demonstrate the long-term benefits of implantation in patients with high-frequency sensorineural hearing loss, this report provides 5-year follow-up on a group of implant recipients who were subjects of the Cochlear™ Nucleus® Hybrid™ L24 Implant System* pivotal clinical study.
Methods: The results of three related clinical studies were compiled to provide outcome data after 1, 3, and 5 years of implant use in a group of subjects who presented with preoperative high-frequency hearing loss and were implanted with a Nucleus Hybrid L24 (Cochlear Ltd., Sydney, Australia) cochlear implant. A subset of the 50 adult subjects (N532) who participated in the Hybrid L24 pivotal Investigational Device Exemption (IDE) completed comprehensive evaluations at 12 months postactivation, 3 years postactivation, and then as part of a postapproval study at 5 years postactivation. Testing included audiometric, speech perception, and subjective satisfaction measures.
Results: Mean unilateral speech perception performance was significantly improved at all postoperative intervals compared to preoperative best-aided results and has remained stable to 5 years postactivation. Ninety-four percent of subjects had measurable hearing, and 72% continued to use electric-acoustic stimulation in the implanted ear after 5 years of implant use. Subjective satisfaction results support objective performance improvements.
Conclusion: Results demonstrate long-term success of patients with high-frequency hearing loss following Hybrid L24 (Cochlear) cochlear implantation. Benefits include speech perception abilities significantly better than those in the preoperative best-aided condition, with additional benefit in those using electric-acoustic stimulation in the implanted ear.
